Iovance lifileucel combo hits 62.5% ORR in frontline melanoma
Iovance Biotherapeutics has presented final Phase 2 data showing that a single infusion of lifileucel combined with pembrolizumab achieved a 62.5% confirmed objective response rate and a 29.2% complete response rate in patients with previously untreated advanced melanoma. Median overall survival was not reached at a median study follow-up of 50.9 months, a result the company says reinforces its ongoing Phase 3 programme.
The data were delivered in a Proffered Paper oral session at the ESMO Congress 2026 in Madrid on 23 October. The analysis drew on Cohort 1A of the multicohort IOV-COM-202 trial, which enrolled 24 patients treated with one-time lifileucel plus up to 24 months of pembrolizumab monotherapy.
Trial data in detail
Beyond the headline response figures, the dataset showed a disease control rate of 83.3% and a median progression-free survival of 19.8 months. Median duration of response was not reached at a median follow-up of 26.9 months for that endpoint, with individual responses running from 4.3 months to more than 57 months. Notably, median PFS after a second line of therapy was 34.3 months, indicating that patients who progressed were still able to receive further treatment, including additional immune checkpoint inhibitor therapy.
Safety was described as consistent with the known profiles of both agents separately, with late adverse events aligned with continuous pembrolizumab monotherapy rather than the more complex toxicity profile associated with dual checkpoint combinations. Translational data from T-cell receptor sequencing detected clones from the infused lifileucel product in peripheral blood for up to 12 months post-infusion, lending biological plausibility to the durability of response observed.
Andrew Furness, Consultant Medical Oncologist at The Royal Marsden NHS Foundation Trust and principal investigator in the trial, said the results provided "a strong scientific rationale to investigate earlier treatment with lifileucel" in the Phase 3 setting.
Market context and competitive landscape
The frontline advanced melanoma setting is dominated by combination immune checkpoint inhibitor regimens, primarily nivolumab plus ipilimumab and, more recently, combinations incorporating anti-LAG-3 agents. Despite the therapeutic progress those combinations represent, Iovance cites published data indicating that roughly half of patients do not achieve durable long-term benefit, a gap the company estimates corresponds to approximately 70,000 addressable patients annually in global markets.
Lifileucel, marketed as Amtagvi, already holds FDA approval for previously treated advanced melanoma and is available at close to 100 authorised treatment centres in the United States. Expanding the label to frontline use would represent a substantially larger commercial opportunity and would position TIL-based therapy alongside, rather than after, established checkpoint regimens. The randomised TILVANCE-301 Phase 3 trial, which compares lifileucel plus pembrolizumab against pembrolizumab alone and is enrolling at more than 80 sites across North America, Europe, and Asia-Pacific, will also serve as the confirmatory trial for full US approval in the previously treated setting.
Iovance will present additional data at ESMO on 26 October, including the first clinical results for lifileucel in soft tissue sarcomas, signalling ambitions to broaden the TIL platform beyond melanoma. The sector is watching those data closely, as durable responses in a second solid tumour indication would strengthen the case for TIL therapy as a platform rather than a single-indication product.
